In order to report a change in income, please complete the Tenant Change Form, and submit it to the Punta Gorda Housing Authority office. The Tenant Change Form must include supporting documentation in order for the change to be completed. You can submit the Tenant Change Form by:
- EMAIL, please take a picture or clear copy of the Tenant Change Form and documentation and email it to pgha@puntagordaha.org
- MAIL the Tenant Change Form and documentation to our office located at:
- 340 Gulf Breeze Ave., Punta Gorda, FL 33950
- FAX the Tenant Change Form and documentation to the PGHA office at:
- (941) 639-1753
- DROP OFF the Tenant Change Form and documentation at the PGHA office in the drop box located next to the Reception desk.
